PARTICIPANT RELEASE AND WAIVER OF LIABILITY *

By checking the box below, I, the parent or legal guardian, acknowledge that I have read and agree to these terms of agreement for my child’s participation in dance classes, rehearsals, performances, and any other activities associated with Mrs. Marcelle’s School of Dance. ASSUMPTION OF RISK: I understand that dance is a physical activity that involves the risk of injury, including but not limited to sprains, strains, fractures, and other serious injuries. I acknowledge that my child’s participation in dance classes and related activities is voluntary, and I assume all risks associated with such participation. WAIVER AND RELEASE: In consideration of my child’s participation in the Studio’s activities, I, on behalf of myself, my child, successors, and assigns, release, discharge, and hold harmless Mrs. Marcelle’s School of Dance, its owners, instructors, staff, agents, volunteers, and affiliates from any and all claims, liabilities, damages, or losses arising from or related to my child’s participation in the Studio’s programs, including any claims related to personal injury, property damage, or illness. PHOTO/VIDEO RELEASE: I grant permission for the Studio to take photographs and videos of my child during classes, rehearsals, and performances for use in promotional materials, social media, and the Studio’s website. I understand that no compensation will be provided for such use. If I wish to opt out, I will notify the Studio by email – mrsmarcelleschoolofdance@gmail.com. AGREEMENT TO TERMS: By checking the box below, I confirm that I have read, understand, and agree to this participation agreement.
